menangkal rasa sakit pada haid

Hampir separuh populasi wanita dewasa mengalami sindrom pra-menstruasi alias PMS (pre-menstruation syndrome). Gejalanya sangat beragam dan acap kali berbeda antara penderita yang satu dengan yang lain. Ada empat tipe PMS yang masing-masing memiliki gejalanya sendiri. PMS Anda termasuk tipe yang mana? Simak pula diet tepat untuk mencegah sindrom yang menjengkelkan ini.

Setiap menjelang haid, selalu saja Murti merasa pusing dan gampang marah. Sampai-sampai pekerjaan kantornya terganggu kalau ia sedang didera gejala-gejala yang menjengkelkan itu. Yang lebih membuat dongkol, teman-temannya jarang merasakan gejala demikian.Gangguan kesehatan berupa pusing, depresi, perasaan sensitif berlebihan sekitar dua minggu sebelum haid biasanya dianggap hal yang lumrah bagi wanita usia produktif. Sekitar 40% wanita berusia 14 – 50 tahun, menurut suatu penelitian, mengalami sindrom pra-menstruasi atau yang lebih dikenal dengan PMS (pre-menstruation syndrome). Bahkan survai tahun 1982 di Amerika Serikat menunjukkan, PMS dialami 50% wanita dengan sosio-ekonomi menengah yang datang ke klinik ginekologi.
PMS memang kumpulan gejala akibat perubahan hormonal yang berhubungan dengan siklus saat ovulasi (pelepasan sel telur dari ovarium) dan haid. Sindrom itu akan menghilang pada saat menstruasi dimulai sampai beberapa hari setelah selesai haid.
Penyebab munculnya sindrom ini memang belum jelas. Beberapa teori menyebutkan antara lain karena faktor hormonal yakni ketidakseimbangan antara hormon estrogen dan progesteron. Teori lain bilang, karena hormon estrogen yang berlebihan. Para peneliti melaporkan, salah satu kemungkinan yang kini sedang diselidiki adalah adanya perbedaan genetik pada sensitivitas reseptor dan sistem pembawa pesan yang menyampaikan pengeluaran hormon seks dalam sel. Kemungkinan lain, itu berhubungan dengan gangguan perasaan, faktor kejiwaan, masalah sosial, atau fungsi serotonin yang dialami penderita.
Sindrom ini biasanya lebih mudah terjadi pada wanita yang lebih peka terhadap perubahan hormonal dalam siklus haid. Akan tetapi ada beberapa faktor yang meningkatkan risiko terjadinya PMS. Pertama, wanita yang pernah melahirkan (PMS semakin berat setelah melahirkan beberapa anak, terutama bila pernah mengalami kehamilan dengan komplikasi seperti toksima). Kedua, status perkawinan (wanita yang sudah menikah lebih banyak mengalami PMS dibandingkan yang belum). Ketiga, usia (PMS semakin sering dan mengganggu dengan bertambahnya usia, terutama antara usia 30 – 45 tahun). Keempat, stres (faktor stres memperberat gangguan PMS).
Kelima, diet (faktor kebiasaan makan seperti tinggi gula, garam, kopi, teh, coklat, minuman bersoda, produk susu, makanan olahan, memperberat gejala PMS). Keenam, kekurangan zat-zat gizi seperti kurang vitamin B (terutama B₆), vitamin E, vitamin C, magnesium, zat besi, seng, mangan, asam lemak linoleat. Kebiasaan merokok dan minum alkohol juga dapat memperberat gejala PMS. Ketujuh, kegiatan fisik (kurang berolahraga dan aktivitas fisik menyebabkan semakin beratnya PMS).
Tipe dan gejalanya
Tipe PMS bermacam-macam. Dr. Guy E. Abraham, ahli kandungan dan kebidanan dari Fakultas Kedokteran UCLA, AS, membagi PMS menurut gejalanya yakni PMS tipe A, H, C, dan D. Delapan puluh persen gangguan PMS termasuk tipe A. Penderita tipe H sekitar 60%, PMS C 40%, dan PMS D 20%. Kadang-kadang seorang wanita mengalami gejala gabungan, misalnya tipe A dan D secara bersamaan.
Setiap tipe memiliki gejalanya sendiri. PMS tipe A (anxiety) ditandai dengan gejala seperti rasa cemas, sensitif, saraf tegang, perasaan labil. Bahkan beberapa wanita mengalami depresi ringan sampai sedang saat sebelum mendapat haid. Gejala ini timbul akibat ketidakseimbangan hormon estrogen dan progesteron: hormon estrogen terlalu tinggi dibandingkan dengan hormon progesteron. Pemberian hormon progesteron kadang dilakukan untuk mengurangi gejala, tetapi beberapa peneliti mengatakan, pada penderita PMS bisa jadi kekurangan vitamin B₆ dan magnesium. Penderita PMS A sebaiknya banyak mengkonsumsi makanan berserat dan mengurangi atau membatasi minum kopi.
PMS tipe H (hyperhydration) memiliki gejala edema(pembengkakan), perut kembung, nyeri pada buah dada, pembengkakan tangan dan kaki, peningkatan berat badan sebelum haid. Gejala tipe ini dapat juga dirasakan bersamaan dengan tipe PMS lain. Pembengkakan itu terjadi akibat berkumpulnya air pada jaringan di luar sel (ekstrasel) karena tingginya asupan garam atau gula pada diet penderita. Pemberian obat diuretika untuk mengurangi retensi (penimbunan) air dan natrium pada tubuh hanya mengurangi gejala yang ada. Untuk mencegah terjadinya gejala ini penderita dianjurkan mengurangi asupan garam dan gula pada diet makanan serta membatasi minum sehari-hari.
PMS tipe C (craving) ditandai dengan rasa lapar ingin mengkonsumsi makanan yang manis-manis (biasanya coklat) dan karbohidrat sederhana (biasanya gula). Pada umumnya sekitar 20 menit setelah menyantap gula dalam jumlah banyak, timbul gejala hipoglikemia seperti kelelahan, jantung berdebar, pusing kepala yang terkadang sampai pingsan. Hipoglikemia timbul karena pengeluaran hormon insulin dalam tubuh meningkat. Rasa ingin menyantap makanan manis dapat disebabkan oleh stres, tinggi garam dalam diet makanan, tidak terpenuhinya asam lemak esensial (omega 6), atau kurangnya magnesium.
PMS tipe D(depression) ditandai dengan gejala rasa depresi, ingin menangis, lemah, gangguan tidur, pelupa, bingung, sulit dalam mengucapkan kata-kata (verbalisasi), bahkan kadang-kadang muncul rasa ingin bunuh diri atau mencoba bunuh diri. Biasanya PMS tipe D berlangsung bersamaan dengan PMS tipe A, hanya sekitar 3% dari selururh tipe PMS benar-benar murni tipe D.
PMS tipe D murni disebabkan oleh ketidakseimbangan hormon progesteron dan estrogen, di mana hormon progesteron dalam siklus haid terlalu tinggi dibandingkan dengan hormon estrogennya. Kombinasi PMS tipe D dan tipe A dapat disebabkan oleh beberapa faktor yaitu stres, kekurangan asam amino tyrosine, penyerapan dan penyimpanan timbal di tubuh, atau kekurangan magnesium dan vitamin B (terutama B₆). Meningkatkan konsumsi makanan yang mengandung vitamin B₆ dan magnesium dapat membantu mengatasi gangguan PMS tipe D yang terjadi bersamaan dengan PMS tipe A.
Ada pula kram perut
Pada hari pertama atau satu hari menjelang datang bulan, banyak wanita yang mengeluh sakit perut atau tepatnya kram perut. Gangguan kram perut ini tidak termasuk PMS walaupun ada kalanya bersamaan dengan gejala PMS.
Kram pada waktu haid atau nyeri haid merupakan suatu gejala yang paling sering. Gangguan nyeri yang hebat, atau dinamakan dismenorea, sangat mengganggu aktivitas wanita, bahkan acap kali mengharuskan penderita beristirahat bahkan meninggalkan pekerjaannya selama berjam-jam atau beberapa hari.
Dismenorea memang bukan PMS. Dismenorea primer umumnya tidak ada hubungannya dengan kelainan pada organ reproduksi wanita dan hanya terjadi sehari sebelum haid atau hari pertama haid. Nyeri perut ini juga tidak ada hubungannya dengen PMS yang mulai terasa 10 – 14 hari sebelum haid. Gejala malah hilang begitu haid datang. Kalau dismenorea membaik atau bahkan hilang sama sekali setelah seseorang melahirkan, tidak demikian dengan PMS. Wanita yang pernah melahirkan malah berisiko lebih tinggi menderita PMS.
Untuk mengatasi PMS, biasanya dokter memberikan pengobatan diuretika untuk mengatasi retensi cairan atau edema (pembengkakan) pada kaki dan tangan. Pemberian hormon progesteron dosis kecil dapat dilakukan selama 8 – 10 hari sebelum haid untuk mengimbangi kelebihan relatif estrogen. Pemberian hormon testosteron dalam bentuk methiltestosteron sebagai tablet isap dapat pula diberikan untuk mengurangi kelebihan estrogen.
